Osborn Manor

Osborn Manor is a supported living accommodation in Fareham providing self-contained flats for individuals with a mental health diagnosis. The service is designed to promote independence, recovery, and community inclusion, enabling people to live as independently as possible while receiving tailored support to meet their individual needs. Referrals to the service are made exclusively through the commissioning body at Hampshire County Council.

The service supports individuals to manage their mental health in a recovery-focused way, helping them to understand potential triggers for relapse and the impact that relationships and external influences may have on their wellbeing. Through person-centred support, Osborn Manor aims to minimise the need for residential care, reduce unnecessary inpatient hospital admissions, and lessen long-term reliance on social care interventions.

A key focus of the service is to increase independence by helping individuals develop social networks, build community connections, and access universal services within their local area. The service also works proactively to reduce and mitigate mental health crises, while supporting individuals to maintain stable accommodation and reducing the risk of homelessness associated with mental health difficulties.

Osborn Manor promotes the principles of personalisation, supporting individuals to make informed choices about their care and support, including the use of Direct Payments and Individual Service Funds where appropriate. In addition, the service encourages and supports individuals to gain and retain employment by providing signposting to specialist employment services, volunteering opportunities, education, and training, helping people to achieve their personal goals and improve their overall quality of life. The overall aim of the service is to support individuals on their recovery journey, maximise independence, and enable them to live successfully within their local community.
